Virginia Qui Tam whistleblower case example.
Amidst allegations that defendants billed for hospital outpatient procedures using codes reserved for physician's office visits instead of an outpatient procedure performed at the hospital, and that the defendants collected both a physician's office visit fee and a facilities fee for the same visit, the University of Virginia Medical Center and its physician practice, Healthcare Services Foundation, in November 2001, agreed to pay $3 million to settle allegations that it improperly billed Medicare for services that were provided in hospital outpatient departments.
